As of September 1, 2008, the Embassy of Japan in Thailand would like to announce that the "Questionnaire" comes into effect. In this regard, those who wish to apply for a visa have to submit the said document as part of their application. In the case that an applicant fails to prepare the "Questionnaire" the Embassy will be unable to accept the application.
